PHILADELPHIA - July 8, 2019 - Oak Street Health, a network of primary care centers providing a new model of value-based care for adults on Medicare, will open three new centers in the Philadelphia area this summer. Led by Regional Vice Presidents, Brandon Holler and Lirui Li, Oak Street Health’s local expansion will provide a more equitable, accessible healthcare option. Oak Street Health will accept multiple health plans including traditional Medicare at the new locations.
The Roosevelt Center is located near the Roosevelt Mall in Northeast Philadelphia at 2311 Cottman Ave. Suite 71. The Strawberry Mansion Center is located at 2301 N 29th St. Suite 500 and the Germantown Center is located at 220 W. Chelten Ave. The Roosevelt location will open in July, followed by Strawberry Mansion in August and Germantown in September.
These new centers will increase the total number of Oak Street Health locations to 10 in Philadelphia and 50 throughout the United States. The company plans to eventually hire more than 150 local healthcare workers to support all three locations. In each city Oak Street Health serves, its care teams are deeply embedded into the community, with many of its physicians living in close proximity to the center.

Patients who visit Oak Street Health will have access to transportation to and from the center, individualized treatment plans, longer-than-average appointments and community-centered support that goes beyond symptom treatment. Since its founding in 2013, Oak Street Health has seen a 40 percent reduction in patient hospitalizations and nine out of 10 patients recommend Oak Street Health based on their experience.
