TORONTO – June 22, 2017 – Iatric Systems, Inc., a healthcare technology company dedicated to helping healthcare organizations enhance their IT investments, and Think Research, a leading provider of clinical decision support tools, reached an agreement to bring together integration services from Iatric Systems and Think Research’s MedRec application.
Healthcare and patient safety organizations endorse medication reconciliation as vital to accurate communication of critical patient medication information. It is also recommended as a discharge and transfer planning best practice.
MedRec allows clinicians to indicate whether pharmaceutical interventions should be added, changed or discontinued from admission through discharge. The application is available on Think Research’s cloud-based platform and meets privacy and safety regulations.
As part of the agreement, Iatric Systems will provide integration services between the MedRec solution and a hospital’s electronic health record (EHR), addressing a key challenge related to effective transitions of care. This integration will allow the medication data to flow freely and will also help streamline clinician workflows.
“Because medication reconciliation reaches far beyond the hospital to post-acute care, as well as home, it plays a significant role in not only patient care and safety but also in better managing readmissions. Integration adds value to medication reconciliation and is one step hospitals can take to help reduce readmission rates,” said Buddy Bergeron, Associate Vice President of Business Development for Iatric Systems. “Iatric Systems has more than two decades of integration experience, and we are excited for the opportunity to use our expertise to help hospitals enhance patient care and quality with improved transitions of care.”
Think Research is based in Ontario, Canada, and the relationship with Iatric Systems will be available to both Canadian and American healthcare organizations. The integration will work with a wide variety of EHRs.
“Care transitions offer more opportunities for gaps in care, affecting care safety and quality. Medication reconciliation offers a smoother transition along the continuum of care,” said Brynne Eaton-Auva’a, VP, Business Development at Think Research. “With this integration pathway for Iatric Systems PDI/PDRx customers, Think Research will continue to build on the success of our MedRec application throughout Canada and offer us the opportunity to increase our presence in the US acute-care market.”
