- CareSync announced Wednesday it is tearming up with NextGen Healthcare, a health IT provider, to provide its customers with chronic disease management (CCM) technology and services.
- CareSync CCM is now integrated into NexGen Ambulatory EHR, which enables providers to have virtual access to nurses who perform care coordination services, such as creating, updating and sharing care plans with patients and providers, as required by Medicare's CCM program.
- Through this new solution, CareSync will enable NextGen Healthcare customers to increase patient satisfaction and profits by simplifying performing and billing CCM services for providers.
